The findings, published May 12 in the Journal of Dental Research, is the latest study to demonstrate that visiting the dentist remains safe, with low risk for transmission of the SARS-CoV-2 virus. SARS-CoV-2 spreads mainly through respiratory droplets, and dental procedures are known to produce an abundance of aerosols, which led to fears that saliva during a cleaning or a restorative procedure could make the dentist's chair a high-risk location.
   
“These findings should help us open up our practices, make ourselves feel safe about our environment and, for patients, get their oral and dental problems treated,” said lead author Purnima Kumar, D.D.S., Ph.D., professor of periodontology at Ohio State, in a news release.

“Getting your teeth cleaned does not increase your risk of COVID-19 infection any more than drinking a glass of water from the dentist’s office does,” added Dr. Kumar, a member of the ADA Council on Scientific Affairs.
   
The researchers set out to characterize the microorganisms generated during dental procedures to quantify the contribution from saliva by collecting samples from personnel, equipment and other surfaces reached by aerosols during a variety of dental procedures.
   
What they found was that no matter the procedure or where the condensate had landed, microbes from irrigants contributed to about 78% of the organisms in aerosols while saliva, if present, accounted for 0.1% to 1.2% of the microbes distributed around the room.
   
For the study, the team enrolled 28 patients receiving dental implants, restorations using high-speed drills, or ultrasonic scaling procedures in Ohio State's College of Dentistry between May 4 and July 10, 2020. Researchers collected samples of saliva and irrigant (the water-based cleaning solutions used to flush out the mouth) before each procedure and, 30 minutes after the procedure, aerosol remnants — condensate — from providers' face shields, the patient's bib and an area 6 feet away from the chair.
   
Salivary bacteria were detected in condensate from only eight cases and of those, five patients had not used a pre-procedural mouth rinse. The SARS-CoV-2 virus was identified in the saliva of 19 patients, but was undetectable in aerosols in any of the cases.
   
The findings make sense, Dr. Kumar said: Irrigant dilutes saliva — a "thick, viscous" substance — by an estimated 20- to 200-fold. These findings help to explain the outcome reported in the 2020 Journal of the American Dental Association study that less than 1% COVID-19 positivity rate among dentists.

"Dental surgeons and hygienists are always at the forefront of the war against bacteria in the mouth, and they of course did not feel safe because they are front-line workers surrounded by aerosol," said Dr. Kumar, who has a periodontology practice of her own and was one of the procedure operators in the study.

Hopefully this will set patients’ minds at ease because when you do procedures, it is the water from the ultrasonic equipment that's the primary source of aerosols rather than saliva, making the risk of spreading SARS-CoV-2 infection not high, she said.

The masks, rated FFP2, contain “biomass graphene”, and should no longer be worn as they can cause respiratory issues, the authority said. The masks are being recalled from use. They had been distributed for use in hospitals, pharmacies and other medical centres and situations.

Regional health authorities have sent hospitals a letter that reads: "At this stage, 60.5 million CE marked FFP2 masks have been identified as potentially containing graphene, of which 16.9 million were distributed in 2020.”

SPF said: “[People should] stop using FFP2 masks labelled ‘Biomass Graphene’ [due to a] possible risk linked to the presence of graphene in these masks.”

Biomass graphene is a material widely used in electronics due to its resistance and conductivity. The recalled set of masks were part of a state stock, and had been provided to France by the Chinese manufacturer Shandong Shengquan New Materials (model SNN 200647), said online news site Mediapart.

When contacted by Mediapart, SPF said that the order dated back to April 2020, when face masks were being bulk ordered “during the face mask shortage”.

SPF added that the Chinese manufacturer had nowhere mentioned the “biocidal activity” present in their products (although “Biomass Graphene” was listed). If it had, the masks would not have been bought, SPF said.

It was not clear that the material had a toxic effect on the respiratory system until later, SPF said.

The warning in France comes two months after Canada warned of the same issue, and took certain models of masks off shelves.

At the start of April, the Canadian health ministry “advised Canadians to not wear masks containing graphene, as they could inhale graphene particles, which can be dangerous for health”.

It said: “Preliminary analysis of the available studies...has revealed that the inhalation of graphene particles may cause early lung toxicity in animals. We do not yet know the potential risks of inhalation of these particles from masks to humans, nor the risks to health that may exist.”

In the context of the coronavirus disease 2019 (COVID-19) pandemic, healthcare workers' (HCWs) protection from contamination is based on the wearing of personal protective equipment (PPE) [1,2]. Transmission routes of severe acute respiratory syndrome coronavirus (SARS-CoV-2) include direct transmission (droplet, e.g. cough, sneeze) and contact transmission (contact with oral, nasal, and eye mucous membranes). The presence of SARS-CoV-2 in the environment of COVID-19 patients therefore reinforces the belief that fomites play a role in transmission of SARS-CoV-2; however, the relative importance of this route of transmission compared to direct exposure to respiratory droplets is still unclear. The wearing of face masks has become usual and ubiquitous, not only in hospitals but also in the community [3]. Without necessarily needing them, the general public is over-consuming surgical and filtering facepiece (FFP) masks irrespective of their specificity [4,5]. This may lead to supply shortages for HCWs in the frontline of patient care, as already reported in several countries [5]. In order to rationalize the use of masks and to prevent their shortage, we present the French guidelines on the indications for the use of anti-projection or surgical masks and FFP masks.

The rational use of face masks is reinforced by the World Health Organization (WHO) update published on February 19th, 2020, which specifies indications for wearing masks as follows: in the context of stage 3 of an epidemic (active circulation of the virus) and during a period of difficulty in supplying masks, or even a shortage of masks, a prioritization of the distribution of respiratory masks (surgical masks in particular) may be envisaged in order to prioritize the protection of health professionals and other personnel, ensuring the continuity of patient care in health and social care facilities and in cities [1]. As defined in the ‘influenza pandemic’ plan by the European Centre for Disease Prevention and Control, this rationalization of mask distribution and use requires:

Most real-world research comparing standard face masks with respirator masks has been in the context of influenza or other relatively benign respiratory conditions and based in hospitals. There are no published head-to-head trials of these interventions in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, COVID-19, and no trials in primary or community care settings. Current guidance is therefore based partly on indirect evidence – notably, from past influenza, SARS and MERS outbreaks – as well as expert opinion and custom and practice.

Policy guidance from various bodies (e.g. Public Health England, WHO) emphasises the need to assess the contagion risk of an encounter and use the recommended combination of equipment for that situation. A respirator mask and other highly effective PPE (eye protection, gloves, long-sleeved gown, used with good donning/doffing technique) are needed to protect against small airborne particles in aerosol-generating procedures (AGPs) such as intubation. For non-AGPs, there is no evidence that respirator masks add value over standard masks when both are used with recommended wider PPE measures.

A recent meta-analysis of standard v respirator (N95 or FFP) masks by the Chinese Cochrane Centre included six RCTs with a total of 9171 participants with influenza-like illnesses (including pandemic strains, seasonal influenza A or B viruses and zoonotic viruses such as avian or swine influenza). There were no statistically significant differences in their efficacy in preventing laboratory-confirmed influenza, laboratory-confirmed respiratory viral infections, laboratory-confirmed respiratory infection and influenza-like illness, but respirators appeared to protect against bacterial colonization.

CONTEXT
Concerns have been raised about the limited personal protective equipment (PPE) provided for UK primary and community care staff with some GP surgeries, pharmacies and care homes having very limited provision. We were asked to find out whether and in what circumstances standard masks are putting healthcare workers at risk of contagion compared to respirator masks. A separate review (ongoing) looks at other aspects of PPE.

BACKGROUND
COVID-19 is spread by four means: contact (direct or via a fomite); droplet infection (droplets from the respiratory tract of an infected individual during coughing or sneezing are transmitted onto a mucosal surface or conjunctiva of a susceptible individual or environmental surfaces); airborne (transmission of infectious agents in small airborne particles, particularly during procedures such as intubation); and faeco-oral.1 2  Coughing and sneezing can generate aerosol particles as well as droplets.

This review considers respiratory protective measures e.g. use of face masks as PPE, to reduce droplet and airborne spread.  It should be noted that in one recent laboratory study, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, the virus that causes COVID-19) survived airborne as long as SARS COV-1 (the virus that causes SARS) when artificially aerosolised and persisted longer on some surfaces.3 This finding is relevant because it suggests that deposited particulates may become resuspended i.e. airborne, when disturbed.
